Good story but I don’t agree with this statement

This concept is outdated. So-called pilot error or human error is part of a systemic breakdown, since operators are at the end of the chain where multiple errors occurred. Systems cause crashes, not human operators (see James T Reason on Human Error or Helmreich). In fact, many times human operators have saved airplanes after computer errors, probably the most famous Flight Qantas 72.
